The EasyForm module is below.  You can add your own CSS style classes to give the module a clean and integrated look to match your own site design by editing the module.css stylesheet that ships with the module.  The out-of-the-box styles are set below. To perform setting changes to the module and choose the fields and auto-responders you would like to be displayed, please login as the Demo user:
